Influence of adenosine and regadenoson on left ventricular ejection fraction, myocardial blood flow and hemodynamics in vasodilator 82-Rubidium PET

Introduction Most 82Rubidium-(Rb)-Positron emission tomography (PET) studies for myocardial perfusion, dipyridamole was used as vasodilator. Less data is available adenosine and regadenoson. Purpose Therefore, the aim to evaluate influence regadenoson on left ventricular ejection fraction (LVEF), blood flow (MBF) hemodynamics in vasodilator 82Rb-PET. Methods Consecutive patients (n = 2299) with suspected or known coronary artery disease (CAD) undergoing 82Rb-PET were studied compared according CAD status normal/abnormal PET (abnormal defined summed stress score ≥4). Differences between rest values (LVEF, MBF, hemodynamics) calculated. The threshold LVEF able exclude a relevant ischemia (as by ≥10% myocardium ischemic based SDS score) assessed. Results Rest differed significantly depending scan results. In CAD, rest/stress 68 ± 12% 73 (p < 0.001), prior 60 14% 63 15% 0.001). during increased 5 6% normal 1 8% abnormal Global MBF (rMBF), (sMBF) reserve (sMBF/rMBF) higher (1.3 0.5, 3.3 0.9, 2.6 0.8 1.2 0.4, 0.8, 2.4 ml/g/min, respectively, p versus scans, irrespective (no CAD: 1.4 3.5 2.8 2.5 2.2 0.7; 1.3 3.1 2.7 1.1 2.3 0.7, 0.7 hemodynamic similar stress. Stress ≥70% excluded negative predictive value (NPV) 94% (CI 92-95%). Conclusions Rest/stress LVEF, are lower scans. Adenosine seem have effect hemodynamics. A has high NPV ischemia.
